World Bank considers financing Telangana’s programme for sustainable transformation in healthcare

The Telangana Government’s efforts to improve healthcare service delivery are set to receive a major boost.The World Bank is actively considering financing the ‘Telangana Strategic Vision for Attaining Sustainable Transformation in Health Care Program’, a programme envisaging improvement in utilisation and quality of priority health services in the State. According to the Bank, the programme involved total financing of US $1,641 million. Out of which, the World Bank Group’s finance would be US $477 million (approximately ₹4,500 crore). Negotiations relating to the programme have started and it is likely to be approved in November.The programme represents a strategic investment in human capital and healthy ageing, accelerating progress towards Universal Health Coverage (UHC) in the State. It would provide an opportunity to shift from fragmented, disease-specific programs to an integrated, person centred model of care, leveraging Primary Health Centre (PHC)-oriented systems to strengthen the response to non-communicable diseases (NCDs) and address the unmet needs of adolescents, women, and the elderly.It would advance economic growth by fostering a healthier and more productive workforce, as well as generating skilled employment in the healthcare economy. Recognising the private sector’s significant role in service delivery in the State, the programme would emphasise governance, regulation, and accountability across both public and private providers to improve population health and enhance citizen trust.The Bank said it would leverage its comparative advantage through financing, global knowledge, and experience to support foundational health system reforms. By strengthening institutional capacity, monitoring and evaluation, and innovation through a Program-for-Results (PforR) financing instrument, the program can serve as a model for other states.The overall government health program relevant to these entities is estimated at about US$5.0 billion over six years. Within this envelope, the proposed PforR supports a defined program boundary of approximately US$1.64 billion, representing about 23% of the relevant government program and 13% of the State’s total health budget.The proposed PforR would finance recurrent expenditures linked to service delivery, including salaries, operating costs, medicines, training, and outreach, for primary and secondary care, reproductive, maternal, neonatal, child and adolescent health (RMNCAH+), NCDs, women’s health, adolescent health, and elderly care. Large civil works and major infrastructure investments are however excluded from the programme.The key result areas, according to the document, are strengthened platforms to deliver high-quality care, behavioural interventions for both people and providers and improved governance and accountability. The program is expected to directly benefit approximately 31 million people statewide including 14% adolescents aged 10–19 years, 30% women aged 20–59 years, and 12% older adults aged 60 years and above.
