Coli is commonly used as an indicator of faecal contamination, meaning its detection in drinking water suggests that the water may have come into contact with human or animal faecal matter.WHAT IS E. COLI, AND WHY IS IT FOUND IN CONTAMINATED WATER?Escherichia coli, or E. coli, is a bacterium that normally lives in the human intestine and is usually harmless. However, there are several types of E. coli, and some can cause disease. These include strains that produce toxins and can cause severe diarrhoea, abdominal cramps and, in some cases, bloody diarrhoea and kidney complications.Dr Prapti Gilada, Consultant Microbiologist, Unilabs Diagnostics and Head and Neck Cancer Institute and In-charge, Antimicrobial Resistance Section, People's Health Organisation India (PHO), explains what the presence of E. coli in drinking water means. “The presence of E. coli in drinking water is an important warning sign. Since E. coli is normally found in the intestine, its detection in water indicates contamination with human or animal faecal matter. Faeces can also contain other disease-causing bacteria, viruses and parasites, which may contaminate water at the same time and cause illnesses such as typhoid, cholera, amoebiasis and giardiasis.”This means that detecting E. coli is not simply about the presence of one bacterium. It can point to a broader contamination problem in the water supply.WHAT HAPPENS AFTER YOU DRINK CONTAMINATED WATER? When contaminated water is consumed, disease-causing microorganisms can enter the gastrointestinal tract. Depending on the organism involved, the amount of contamination and the person's health, symptoms can range from mild stomach upset to severe gastrointestinal illness.Dr Gilada explains:“When contaminated water is consumed, disease-causing bacteria and parasites can enter the gastrointestinal tract and cause diarrhoea, abdominal cramps, vomiting and fever. While many infections are self-limiting, severe illness can result in significant dehydration and, in some cases, complications affecting other organs.”Diarrhoea and vomiting can cause the body to lose large amounts of water and electrolytes. If these losses are not replaced, dehydration can develop. This can be particularly dangerous in young children, older adults and people with other health problems.In some infections, the illness may be more severe. Certain strains of E. coli can cause bloody diarrhoea and, in serious cases, complications such as haemolytic uraemic syndrome, or haemolytic uraemic syndrome (HUS), which can affect the kidneys.WHY E. COLI IN WATER IS A BIGGER WARNINGThe presence of E. coli in drinking water is important because it can indicate that contamination has occurred somewhere along the water supply chain.Water can become contaminated at different points, including the source, transportation system, storage tanks, water coolers or taps. Poor sanitation, inadequate cleaning, damaged infrastructure or improper handling can increase the risk.Dr Gilada says:“The key message is that E. coli or coliforms in drinking water is not merely a finding of one bacterium it is an indicator that the water has been exposed to faecal contamination and therefore may not be safe for consumption. Safe drinking water requires proper treatment, clean storage and regular microbiological testing from the source right up to the point where it reaches the consumer.”NOT EVERY PERSON WHO DRINKS CONTAMINATED WATER WILL FALL SICKDrinking water that contains E. coli does not mean that every person who consumes it will necessarily develop an infection. The outcome depends on the particular strain or other microorganisms present, the level of contamination and the individual's susceptibility.Some infections may cause only mild diarrhoea or abdominal discomfort and resolve on their own. Others can cause prolonged diarrhoea, high fever, vomiting or blood in the stool and may require medical attention.This is why the presence of E. coli should be treated as a water safety warning, rather than assuming that everyone exposed will develop the same illness.WHAT SYMPTOMS SHOULD YOU WATCH FOR?After drinking potentially contaminated water, symptoms that may indicate a gastrointestinal infection include: Diarrhoea Abdominal cramps Vomiting Nausea Fever Weakness Blood in the stool in some infectionsThe most immediate concern with significant diarrhoea or vomiting is dehydration. Reduced urination, extreme weakness, dizziness, unusual sleepiness, very dry mouth or difficulty keeping fluids down can be signs that medical attention is needed.People should seek medical care if symptoms are severe, persistent, associated with blood in the stool, significant dehydration or high fever.WHO IS MORE VULNERABLE?Although anyone can develop a waterborne infection, some groups are more vulnerable to severe illness and dehydration.Young children, older adults and people with weakened immune systems may be at greater risk of complications. Pregnant women and people with certain underlying health conditions may also need greater caution if they develop significant gastrointestinal symptoms.For these groups, prolonged diarrhoea or vomiting should not be ignored because dehydration can develop more quickly.HOW CAN CONTAMINATED WATER BE PREVENTED?Safe drinking water requires more than testing the water once. The entire chain needs to be monitored, from the source to storage and finally to the point where the consumer drinks it.Proper water treatment, hygienic storage, regular cleaning of water coolers and tanks, maintenance of pipelines and regular microbiological testing can help reduce the risk of contamination.For railway stations and other public places, ensuring that drinking water facilities are properly maintained is particularly important because large numbers of people use them every day.The finding of E. coli in drinking water is ultimately a reminder that water quality and sanitation are closely connected.
