Could gum disease be linked to Alzheimer's? What a study found

A 2019 study found evidence of a bacteria linked to gum disease and its toxic enzymes in the brains of people with Alzheimer's disease. But does this mean gum disease causes Alzheimer's? That is the question raised by a widely discussed study published in the journal Science Advances. Researchers investigated Porphyromonas gingivalis, a bacteria strongly associated with gum disease, and found evidence of the bacteria and its toxic enzymes in brain tissue from people with Alzheimer’s disease.The finding attracted attention because it suggested that Alzheimer’s disease may involve more than the buildup of proteins in the brain. The researchers proposed that infection and inflammation could play an important role in the disease. However, social media posts about the research have often presented the findings as proof that Alzheimer’s is simply a bacterial infection. That conclusion is not supported by the study.Here is what researchers actually discovered and what it could mean.WHAT DID THE STUDY FIND?The study focused on Porphyromonas gingivalis, a bacteria associated with chronic periodontitis, a serious form of gum disease. The researchers examined brain samples from people with Alzheimer’s disease and looked for gingipains, toxic enzymes produced by the bacteria. They found RgpB gingipain in 96 per cent of the Alzheimer’s brain samples examined and Kgp gingipain in 91 percent. The researchers also detected P. gingivalis DNA in Alzheimer’s brain tissue and cerebrospinal fluid from some living patients.This is where the often repeated claim that the bacteria found in about 90 per cent of Alzheimer’s brains comes from. However, it is more accurate to say that researchers detected the bacteria’s toxic enzymes in more than 90 per cent of the Alzheimer’s brain samples tested. It does not mean that 90 per cent of patients had large amounts of living bacteria growing inside their brains.HOW COULD A MOUTH BACTERIA REACH THE BRAIN? P. gingivalis normally lives in the mouth and is strongly linked with gum disease. When gum tissue is damaged or inflamed, bacteria can enter the bloodstream. Researchers have suggested that bacteria or bacterial products could potentially travel to other parts of the body, including the brain.The study proposed that gingipains may damage important proteins and contribute to inflammation and damage to nerve cells. The researchers also found a connection between gingipain levels and certain Alzheimer’s-related changes involving tau and ubiquitin.WHAT HAPPENED IN MICE?The researchers also conducted experiments in mice. After oral infection with P. gingivalis, the bacteria were detected in the brain. The infected mice showed increased production of amyloid beta, a protein associated with Alzheimer’s plaques, along with inflammation and damage to neurons.Researchers then tested drugs designed to block gingipains. In the experiments, blocking these enzymes reduced the bacterial load, lowered inflammation, reduced amyloid beta production and protected some neurons in the hippocampus.These findings were important because they provided a possible biological explanation for how an infection could contribute to brain damage. But experiments in mice cannot prove that the same process causes Alzheimer’s disease in humans.DOES THIS PROVE GUM DISEASE CAUSES ALZHEIMER’S?No.This is the most important point to understand. The study provided evidence for a possible connection between P. gingivalis and Alzheimer’s disease, but it did not prove that the bacteria is the main cause of the disease.Alzheimer’s is a complex condition involving several biological processes. Genetics, abnormal proteins, inflammation, blood vessel health and other factors can all play a role.There was also a major test of the idea in humans. A drug called atuzaginstat was developed to block gingipains and was tested in a Phase 2 and Phase 3 trial involving 643 people with mild to moderate Alzheimer’s disease. The trial did not meet its main cognitive and daily functioning goals in the overall group.WHY THE FINDING STILL MATTERSThe research does not prove that brushing your teeth can prevent Alzheimer’s disease. It also does not mean that everyone with gum disease will develop dementia.However, it has opened an important area of research into how infections, inflammation and brain health may be connected. Scientists continue to investigate whether treating gum disease and controlling oral bacteria could have wider health benefits.For now, the strongest conclusion is simple. A bacterium linked to gum disease has been detected in Alzheimer’s disease research, and scientists are investigating whether it contributes to brain damage.
